America First Healthcare: From a Tracking Failure to 218 Booked Appointments a Day

America First Healthcare books thousands of appointments a month from paid social and search. In mid-August 2026 a tracking failure cut booked appointments by 42 percent in two days. Megawebvision's CHIEF Signal report caught the drop on the day it happened, quantified it, and the fixes that followed took the operation to a monthly total of 11,240 booked appointments and a daily high of 218.

Situation

America First Healthcare runs a high-volume lead operation for Medicare Advantage plans: Meta and Google campaigns feed a HighLevel CRM with 260 booking calendars, and a sales floor works the appointments. At that volume, leadership cannot see a problem by looking at the dashboard. They need a daily read that says what changed and why.

Megawebvision built that read: CHIEF Signal, a daily growth-intelligence report that pulls Meta spend, HighLevel bookings and GA4 into one edition and sends it to the leadership group before the day starts. Edition 001 went out on August 12, 2026.

The constraint

Two days after the first edition, the numbers broke. From August 12 to August 14, spend fell 14.3 percent, clicks fell 22.5 percent, leads fell 32.9 percent and booked appointments fell 41.8 percent, from 146 to 85 a day. Spend down a little and bookings down a lot is not a demand problem. It is a tracking and delivery problem downstream of the click.

The Signal report flagged it the same day. The incident recommendation to the CMO on August 15 named the tracking failure, separated it from a campaign issue, and set the rule the team kept afterwards: do not treat a rebound as a recovery until the trailing window confirms it.

What was done

The work stayed on the constraint: measurement and attribution, then the waste the measurement exposed.

  • Daily CHIEF Signal editions with a trailing 28-day comparison, so a one-day spike could not be mistaken for a trend. By August 20 the trailing window read 1,237 booked appointments from 6,333 leads.
  • 885 sale-to-lead attribution links repaired in production, with before and after states verified in the Command Center.
  • 112,422 HighLevel opportunity records reconciled and 91,447 contacts matched back to the consumer ads that produced them.
  • Google Ads geo-targeting corrected after the data showed 94.2 percent of generic Search spend was being attributed outside the US target. Presence-only targeting was applied and verified on September 7.

Readout

Booked appointments recovered from the August 14 low of 85 to 170 on August 17 and 171 on August 19, then reached a daily high of 218 on August 31, verified from a fresh Meta and HighLevel read. August closed at 11,240 booked appointments.

The number leadership now watches is not bookings. It is the trailing-window comparison in the daily edition, because that is the number that told them the truth on August 14. Booked is not attended, and the report says so in every edition.

With attribution repaired, the next question became answerable: which campaigns produce appointments that turn into sales, not just appointments. That is the work of the following quarter.
